Fill out COVID-19 Immunization Consent Form at the Pharmacy when you show up for your appointment. If you have insurance, please bring your insurance card.
Show up to your appointment at your scheduled time, if you need to cancel, please call
(208) 742-DRUG or reschedule through the email confirmation sent when appointment was scheduled.
